Here's how I did it. Saving it to the computer was a hassle....I didn't know how to convert it to form....so I copied then pasted the whole thing into an email and replaced the blanks with bold text of my info...it worked very easily. I then attached two photos to the email and sent it off to the email address they gave (which was strikingly similar to mine). Kelly C.
