Thanks to everyone who responded. David Hanks and Ed Riggins were both able to tell me the correct connections. After many emails off list between Ed and I last night, we figured out what caused this confusion. M&H wiring harness seems to be manufactured correctly, but the AC Delco $50 horn relay terminal had the raised "key" stamped onto the wrong terminal (on the unused #4 terminal).
 
I verified our findings this morning by ohming out the relay and then hooking it up to a battery for proper relay operation.
 
One step closer to bringing this car back to life...

It's a picture of my brand new, upside down  $50 horn relay (subject for another day). I need help with the connections. Here's what I think I know. Please correct me if I'm wrong, and fill in the spots I don't know...
 
red wire - 12vdc goes to battery through the junction block, and a few other places. Hooks to terminal 5 (spade) as I have them labeled.
 
black wire with white stripe - goes to fuse block on firewall. Hooks to terminal 3 (spade) as I have them labeled. (tied to 5).
 
double green wires - 12vdc going to the horns when relay is energized. I THINK it goes to terminal 1, but not sure.
 
black wire - I think this is the horn ground, which completes the relay circuit, which applies 12vdc from red wire to the double green. I don't know where this hooks to.
 
When all pictured wires are connected, if my count is right, there will be one terminal unused. What goes there?
