sounds like 2 different problems to me- bad fan clutch, worn belt and/or idler pulley bearing or tensioner pullry bearing.

However, it doesn't really explain why, when the belt "engages" completely (stops slipping) the fan runs nonstop. The fan clutch should not be "engaged" at highway speed.
