I thought some type of fungus or something may be the culprit, which was why I suggested a mange treatment...usually that stuff will kill all living organisms if it is a good one. Fungus and stuff can be pretty much anywhere, including the air. It probably came from a source you will never identify. Once any living organism finds a good breeding ground, such as on a dog, it can multiply and get out of control, where as it had been present all along but not had the proper place to grow. I wouldn't concern myself too much with trying to find out where it came from, because you probably never will. (of course though if it reoccurs and you start noticing a correlation between that and a certain place or something, that is a different story).
Once it is treated and healed, you will probably never see it again. But it will probably take time for all the tale tale signs to be gone.
