After investigation with Jay, it was found that when we are in toolbar
accessibility
mode the top (browser) window is focused (which is never the case
otherwise). When
alt-f4 is pressed, the defprocwindow is supposed to transform the syskey
msg into a
WM_CLOSE. Somehow this does not happen.
