Ian, I disagree. A website developer should just be able to specify that  
they want a file, it shouldn't matter
where it comes from (local hard-drive or web server). I agree that it's  
more efficient to download directly from
the url, but anything that forces sites to change their implementation is  
not terribly practical.

As for the various failure cases you detail, we could design around those.  
One way would be to fetch the file
when the user pastes the url, just like Gmail pre-uploads attachments  
before you hit send. We could then
show a preview before the user uploads it to the server.
