New issue 8836 by SIMSLIM: Tab crashes with 2 similar forms on same page

What steps will reproduce the problem?
1. Have two forms like in the file i attached
2. Open the page
What is the expected result?
To show the 2 forms
What happens instead?
Get a sad-tab
Please provide any additional information below. Attach a screenshot if
possible.
Changing something little in the form will stop the sad-tab. Removing the
action attribute, changing the select element into an input element,
changing a name-attribute, etc.
