Okay, I've been watching very closely, and I've noticed that more often than not it happens after I use the "restore" button to restore tabs after a crash or when I open several new tabs when chrome is already running slow (so the tabs all seem to open at once when chrome "catches up"). I suspect it is this behavior of opening several tabs simultaneously that is causing it. So I created a simple page with six links and middle-clicked on them very quickly several times. Fairly often, two tabs would start out with the same title, but as the pages load the incorrect one (the one on the left) will fix itself. Sometimes it doesn't fix itself, and that is what this bug is (as far as I can tell).
