I opened up the password manager and noticed that the first entry in the list had a blank url. The username matched the one I use for the proxy. I deleted it, restarted Chrome, and tried loading up a site. The proxy authentication dialog came up and I typed in my proxy info. The site loaded up and I got the bar at the top asking to save the password. The empty entry was back in the password manager.
