Just wanted to add that I also observed the issue where the entry for my proxy server was blank but did have the right user name showing. Additionally, the proxy at my corporate environment is tied to an enterprise-wise user system (LDAP), and if there are too many hits against the proxy with the wrong password, it locks the account. I have noticed that even if I delete the proxy entry, restart Chrome, and put in a username and password for the proxy server, I can browse around a few websites, but then after a minute or two of usage (or a certain number of sites I have gone to), my account is locked. This would suggest that under the hood, Chrome is using the wrong password (or perhaps a blank password) against the proxy and it's locking the account.
