Sorry for the slowness in getting back on this issue. It doesn't occur all the time. One thing that seems to consistently reproduce the error, more often than not is leaving Chrome running over a period of time, mounting a .dmg file and trying to copy the contents (usually an .app) to a another directory. As you rightly mentioned, once in this state its v. hard to do anything.
