Hey Jim,
My bad.  The problem was that I did not rename the file extension
to .mpg.  The default file extension for the JVC files is .m2t and when
Cin tried to open the file, it didn't recognize the extension and just
said "<filenames> format could not be determined."  So when I renamed it
to .mpg, Cin created the toc index file automatically and I was able to
edit it.
