There are several patch panel manufacturers. Ortronics is one. Distributors
such as Graybar and Anixter sell them. They also sell telco 25 pair cables
to connect them to the Cat5k card. (Also make an adapter part number "oops"
for when you buy a cable with the wrong end. Been there - done that.)

Not sure about pricing. Have you tried eBay for used?
How will one of those cards help you with certification? Or is it that the
Cat5k that you want to buy also comes with these cards. If so, consider
selling the cards you don't want and buy those you do. Notice that dealers
often sell a chassis with a bunch of cards that aren't useful. They then
sell individually the good ones.
