What problem are you trying to solve?  There are lots of reasons to 
tunnel, which use different protocols depending on the application.

In general, you can establish arbitrary tunnels between customer 
routers, without ISP involvement. I'm not sure I understand what a 
host-to-host tunnel would be -- something like IPsec transport mode 
as opposed to IPsec tunnel mode? IPX in IP from a NetWare server?
