The 3000 VPN series can do almost all the items you
mentioned:
It can autheticate vs. a local table
It can authenticate vs. a Raduis (or is it TACACS, or
both?)
It can authenticate vs. NT domain.
and maybe more.
The unit should sit on your network, with one
interface on the dirty side and the other on the
trusted side. Like your firewall.
The configuration is very simple. It is text based
menu driven. It also has a very usefull GUI (I think
JAVA) that works well.
