The serial int could be connected to a CSU/DSU which might give the int the correct control signal states to make the serial int appear to be up (example would be that the remote CSU/DSU is broken but carrier signal is up on local int). In this case, since it's frame relay, the 2nd up would not occur unless LMI keepalives were occuring. What cisco is saying is UP/UP is just the int status. Doesn't mean that data is being xfered. The most popular way to know that an interface is in use is to watch in/out stats (via sh int). Clear counters is another usefull cmd.
