I have seen problems similar to this, but that was with supervised transfers... I don't have an immediate suggestion, but I'm pretty sure the problem is on the voice gateway. Somehow, the voice gateway isn't registering the fact that the PSTN call hung up. Also, I've never worked with FXO so that might be an issue as well.
