Sounds like it was a physical layer issue:

bad rollover or,
bad db9, or
bad serial port on the computer

Swapping out each of the above, one at a time, undoubtedly led to your
solution....I think.  The other possibility was that your keyboard was
messed up and sending the wrong keystoke combos.
