Perhaps you have only a 2V DSP chip in the router (PN=PVDM-4)!?!? Issue a Show Diag at the Exec Mode prompt, and see if it says DSPs = 2 or 4.
If it's 2, then that answers your question about not being able to configure more then 2 voice ports. But, somehow I would think that if you only had 2 DSPs, then you would only have 2 of the 4 FXS ports initialized. Perhaps you should do a "wr era" (if it's not a production router) and then bounce that bad boy to see if you get different results. You might even see about rolling back to 12.2(4)T and see if that makes a difference.
