As everyone has already said, the OFA certificate is fine.  I send mine to
OFA as it only costs $15 ONE TIME, and rechecks are FREE.

There is a CKCSC form of sorts--this is the one designed by Dr. Buchanon and
group.  It is different from the OFA form in that it shows the shape of a
dog and a heart.  A cardiologist can pinpoint the location of the murmur on
this illustration if there is one.  A nice form, but not necessary.  I am
certain this is the form they were talking about.
