Sarah,
My cat, who is about the same age as the "cousin" you wrote about, doesn't care
where we live so long as I'm there...none of my house cats ever cared.  This one
would actually be even happier if there were no dogs and if we never had
houseguests... but that's a cat for you.
    My main suggestion for a cat who's always been good at using the litter box
and who is now missing it is to check for bladder or kidney stones.  Cats are,
of course, prone to these problems...and if she's having problems passing urine,
she will miss the box more often than not...stones can also make her very, very
uncomfortable...that could be why she isn't eating, etc.
    Cats don't handle most medications well...but I'd suggest to the vet that
maybe she's got a urinary infection or stones or something like that.  The only
cats I ever had who missed the box had stones or some other problem in that
area.
