This suggestion is mostly for Nigel. :)
What about splitting clamav-milter.c into ~4 parts: README/INSTALL, ChangeLog, clamav-milter.h, clamav-milter.c? At this moment, ~30% of source's size are comments and change list.
Wouldn't it be easier to maintain such splitted clamav-milter? IMHO, it improves (at least) readability.
