Your best bet is to download the latest source from CVS, run 'rpm -ev clamav (or whatever the clam RPMs are called that you have) configure --enable-milter --enable-cache make make install' and follow the documentation in .../clamav-devel/clamav-milter/INSTALL. There is one dependancy - you must have the sendmail-devel RPM installed.
