You can _add_ a header with the Header config option, see:
http://memberwebs.com/nielsen/software/clamsmtp/clamsmtpd.conf.html
but you cannot change the Subject header; perhaps what you saw was the result of using MailScanner or something else (for spam filtering), it could be added after clamsmtpd scanned the message and before delivery.
