I apologize if I am being naive. I am new to clamav / linux / sendmail and I am trying to do the legwork so that my friend (he has experience with this kind of stuff) can help me out and I can learn. That said here is my freshclam.conf

Other than the fact that you're not reading the error message or your logs? You've got an error in your freshclam config - you shouldn't be trying to notify clamav milter, but clamd. -- Please keep list traffic on the list.
