You should install clamav-milter-0.88.3-1.el4.rf.i386.rpm and read the
docs that come with it.

Modify your sendmail.mc, adding the following:
INPUT_MAIL_FILTER(`clamav-milter',
`S=local:/var/run/clamav/clamav-milter.sock, F=T,T=S:4m;R:4m;E:10m')
That's supposed to be one line, by the way.
Then run
make -C /etc/mail
chkconfig clamav-milter on, start it and then restart sendmail.
