I am not sure what kind of tools Windows has for this type of thing but you could always log the time it takes to scan each file and adjust your exclude list accordingly or even better you could install some type of file auditing software( tripwire, etc) and just run clamscan on the files that have been added or modified. This would probably require some scripting though. If neither of the above suit you you may want to look at how much memory you have. With only 256M of memory you are probably hitting swap if you have any significantly sized files.
