Well, let's see.  I did install via the ports, and the appropriate entry
in /etc/rc.conf is there (clamav_milter_enable="YES").  The rc.conf
entry that I removed was part of a set of instructions that was written
a while back, and may now be quite out of date.  However, I do also
have a milter flags entry in rc.conf which, among other things, tells
the program where my quarantine directory is located.
