I am not at work today, but the SNMP problem was only part of our issue.
We had database corruption in Clean Access that prevented OOB from
working with the 3750s. I rebuilt the manager & servers by hand after
using Darik's Boot & Nuke to erase the CASs & CAM. (A CCA install does
not wipe all old data.) Since we use HA, I just split the HA CAM.

I am convinced that after upgrading from 4.0.5 to 4.2.1 that there is
again some minor database corruption. I am now hand building a 4.1.2.1
CAM and will upgrade the CASs to migrate to 4.1.2.1.

Is this a new 4.1.2.1install or upgraded? At least part of our initial
corruption issue was a result of custom jsp login pages.  We stopped
doing that :) I think our current corruption is a result of the database
upgrade. There are several minor issues pointing to corruption.


Maybe you can get a demo license to test if there is still a problem
with a fresh install. I am fortunate to have a CCA system in our test
lab for experimentation & testing.


The IOS SNMP problem on a stack with more than one switch, would change
the vlan on the wrong port, so the user would not get moved OOB. Is that
similar to your problem?

We are running 12.2(40)SE on all our 3750s (WS-C3750-48TS).  We upgraded
to
12.2(40)SE from 12.2(25)SE in early January as a result of the two
previous
general SNMP failures, but have had 3 more re-occurrences since.

We use a central deployment of CCA (4.1.2.1) in OOB virtual gateway mode
. 
We plan to upgrade to 4.1.3 soon.
