I ran into a similar situation.  The client sends the ACK to the URI in the 
top-most Record-Route header that is in the 200 OK response to the INVITE 
transaction.  in my case, that was an FQDN that my client could not resolve 
because of DNS limitations in my test environment.  If the client cannot route 
to that URI you may need to re-configure things so that it is an IP address - 
that was my work-around.
