I'm on vmware.
I've managed to recover from my crashed cloudstack so I can give another go to 
the 3.0.2->4.0 upgrade.

I'm really not sure that anything has changed. Did you use KVM or Xen? With KVM 
at least, the systemvm.iso used is on the agent, something like 
/usr/lib64/cloud/common/vms/systemvm.iso
