Lets make a single page, no pop-up, as the ldap configuration is just one set
of data:
hostname:
searchbase
queryfilter
binddn
bindpass : <password field>
port: <integer>
<checkbox> ssl return true/false;
If above is checked enable following fields:
truststore
truststorepass
