schema-40to410.sql should run as part of 40 to 41 upgrade. This file contains 
sql to add region_id column.

ALTER TABLE `cloud`.`domain` ADD COLUMN `region_id` int unsigned NOT NULL 
DEFAULT '1';

can you verify that schema-40to410.sql was executed?
