An older version of CloudStack documentation had Troubleshooting sections: one 
in the Install Guide, and another in the Admin Guide. These were not brought 
forward to the 4.0 documentation, as questions arose about whether they were up 
to date and useful. The Troubleshooting section of the Installation Guide was 
proposed as an XML patch, but never committed due to reviewer issues (see 
https://reviews.apache.org/r/7645/). The Troubleshooting section of the Admin 
Guide was never converted to XML. It is attached to this bug in its original 
file format. It would be useful to take the non-controversial parts of the 
original patch, plus any useful info from the attached file, and use it as a 
start to a newTroubleshooting section.
