Fix For: 4.2.0
Add support for a new field type, 'date,' which is a text field that on-click
will provide a jQuery UI calendar drop-down. This will populate the text field
with a standard format date.
This requires a) implementation of jQuery UI's datepicker widget, and b)
styling of the widget
