Since there is no schema change for versions 4.0.1 and 4.0.2, the fix is to
reuse the existing db upgrade class Upgrade40to41 for both 4.0.1 and 4.0.2
source db versions.
Applied on master, review sent for 4.1 branch which would release 4.1.0:
Upgrade40to41
