Good ideas so far! As jclouds supports cloudstack, tools that work on cloudstack do, too.
Pallet, for example, supports what you mention, inc MySQL (github/pallet), and it is an OSS tool. enStratus is a service and has functionality inc what you mention, and this also supports cloudstack. Scalr also supports cloudstack, and is either a service, or you can install it (OSS). The fact you mention DB setup actually cuts out a lot of other good tools!
