there was an ISO still attached to the original VM when it was exported.

hypervisors often make VHDs that are not compatible with other
hypervisors. We have seen problems with importing VHDs generated by
Hyper-V.
You can try running the VHD through different tools (e.g. XenConvert,
XenCenter) to see any can import it to XenServer successfully. If you
can get it into XenServer, export the VHD from XenServer and try
importing the resulting VHD to CloudStack. Otherwise I suggest using
XenServer to build templates.
If you built this template in XenServer, how did you obtain the VHD from
the XenServer environment? The VHD may have been dependent on another
VHD, or somehow corrupted. I suggest exporting VHDs from XenCenter
unless you are absolutely sure you can get a valid, standalone VHD
directly from an SR.
