I think there are only a few of us who make this mistake (myself included).  
Usually because we use email extensively and sometimes get distracted when 
replying to a message, and oops - missed a step....

As long as we all make an effort to try and prevent this, I personally don't 
see a need to do anything special with the mailing list.  If/when it happens, 
the author of the message is ultimately responsible - not the list or the 
list software.
