Try switching to a different back end.  Likely the problem is not with Amarok 
but with the audio engine it is using.  

I have had good luck with the xine engine as well as gstreamer.   

One thing to also check is that artsd is not running.  Do this from the KDE 
settings panel (Sound & Multimedia -> Sound system) by unchecking the "Enable 
Sound System" option.

This is still a sticky area, hopefully arts will be replaced with something 
better in the near future.

The technology is there (ALSA, jack specifically) but more work needs to be 
done by the distros to enable and configure this stuff.

Also, what distro and kernel version are you using?
