I've seen the http://gallery.menalto.com/ (they call it "Gallery") used in a few places. It has some modules to tie it into existing Content Management Systems (Drupal for one). It also has a feature I have rarely seen in online apps - the ability to upload multiple images at once into a gallery (upload a zip of images, and it'll place all the images into the same gallery with similar properties/metadata.) From what I've seen, and my one trial of it, it looks pretty good.
