Ssh should start automatically, it has on every distro I have used.
The answer depends on which distro you are using.
Hmm odd, never had it start automatically *ever* and I'm a huge distro-hopper.
I'm using Kanotix which is basically Knoppix on steroids which is basically Debian on steroids.
