you're probably missing a [L] somewhere and the URL is getting processed more 
than once. or you have it actually redirecting somewhere so it runs the chain 
twice. and it looks like you aren't checking against the url pointing at 
index.php directly already.

but it's hard to say because once again you don't provide enough information 
to really be able to help you.

to put it simply, because you're doing something wrong. if you would simply 
attach the vhost config or point to a URL where it could be looked at (hint: 
pastebin.com) you might get definitive answers.
