There is a package called build-essentials, so all you need to do is install it and kernel-package (and the libncurses-dev if you want menuconfig). Instead of bothering with append-to-version, just edit the /etc/kernel-pkg.conf with the appropriate values. There is also /etc/kernel-img.conf which has some options for generating kernel images. I almost never modify this file as the defaults are fine 99% of the time.
