I have this card and a couple of its predecessors (model 8506-8 to be specific) in production with stock 2.6 kernels. This is a small sample, but I have yet to need to update the drivers.
The obvious question to me is: have you checked the drive itself for errors? Also the 3ware utility tw_cli can provide some info. From within this program, what does the alarms command return? Again, my first inclination would be to replace the disk itself. Do you see any errors in dmesg etc.?
