We do a hardcopy careplan (in a word
document) once a year, then annotate with updates as needed. We
thought this was better because it showed that we kept a dynamic careplan.
Now I am wondering if this is really the best approach. Does everyone else
update their careplans (print out hardcopy) more often, what do you think.
