To whoever wrote about VB4,   I deleted your email by mistake.  If you read the clarification on VB4 in the manual, it states “The date on which a staff member completes the care planning decision column (Vab), which is done AFTER THE CARE PLAN IS COMPLETED.  The care plan must be completed within 7 days of the completion of the comprehensive assessment (MDS and RAPs) as indicated by the date in VB2.”   I usually date this as the same day of my care plan as I ensure that all of the applicable raps are care planned appropriately.    Brenda W.
