The past 15 months of MDS's, RAP's must be on the current chart or available in a convenient place for staff to reference at any time.  At one home I worked, they kept the past current comprensive and the quarterlies on the chart, and the rest in a 3 ring binder.  The other homes used filing cabinets, etc.  They cannot be kept in a room that is locked at any time.

If a res goes home and comes back, we are supposed to bring the past 15 months assessments on the new chart.  Does this include the RAP's?  That makes a lot of paper!
