We have the MD sign both the therapy plan of care and the therapy clarification orders because we need signatures in both places to meet the requirements for the state Medicaid assessment (New York state, the PRI -- Patient Review instrument).

sign their evaluation. We have them sign clarification orders stating what
modalities they are doing with them and the frequency and duration. We have
them sign the PT and the OT plan of care portion of their evaluation also
but is this more than we need to do. I don't want to be set up for "one
more piece of paper that isn't signed".
