I'm not Rena, but what I would do is make a note documenting the error in coding the PPS assessment. Then do a modification of the assessment, indicating either data entry or item coding error, whichever is true. (Did you know the pu's were there and just hit the wrong keys, or were you unaware they were still being treated?"
 
If the assessment was accurate and the nurses were "copycat" charting, then I would have the nurses involved write a late entry dated today, indicating that no treatment was provided as was documented.
 
You may still be cited, but with the modification/corrected charting it should be minor.

Can we be cited if a mistake is made in coding a PPS only assessment? A resident with a stage 2 pu was mistakenly coded as o's in M1 and as a 1 in M2 ( as if the pu was healed) when the rx record showed she still had the pu.What should I do?
